Introducing Chronic Disease Education Program – for Diabetes & High Cholesterol

Woodbridge Med Walk-in Clinic is excited to announce that we are introducing a new chronic disease education program with a focus on diabetes and high cholesterol.

Chronic disease is the leading cause of death and disability in Ontario. Chronic conditions left unmanaged can predispose and deteriorate individuals to other chronic conditions. However, with the right management and lifestyle choices, there is great potential to improve health outcomes and reduce the burden of chronic disease.

What to Expect

Our program will be led by a registered dietician with assistance from other members of our healthcare team at our clinic location. The program will teach individuals how to manage their diabetes or high cholesterol through:

  • Healthy eating
  • Physical activity
  • Stress management
  • Foot care (annual foot screening)
  • Self monitoring
  • Proper medication use
